Risk assessment of vascular thrombosis due to coronavirus
Coronavirus infection is associated with an increased risk of vascular thrombosis. The following questions are used to assess whether you are at increased risk of vascular thrombosis. Please note that the questions here are intended for screening a person with a COVID-19 infection who is able to manage at home, and you may not need heparin medication despite risk factors.
1. Do you take any medication to prevent blood clotting?
If you use any of the medication listed below to prevent blood clotting (trade names in brackets), you have protection against the risk of a thrombosis caused by coronavirus infection.
You can stop the assessment here.
- So-called blood thinners (Marevan, Eliquis, Lixiana, Pradaxa, Xarelto).
- Subcutaneous heparin therapy (For example: Fragmin, Innohep, Inhixa, Ghemaxan, Enoxaparin Becat).
2. Are you pregnant, or have you given birth or had a caesarean section in the past 6 weeks?
If you are pregnant or have given birth or had a caesarean section less than 6 weeks ago and you have the risk factors listed below, go to the COVID-19 page matching your home municipality's wellbeing services county for instructions and contact information for potentially starting heparin medication. The assessment for heparin medication needs to be completed within a few days.
Do you have a condition that could put you at risk of thrombosis?
- Your pregnancy has lasted less than 28 weeks, and you have 4 or more of the risk factors listed below
- Your pregnancy has lasted 28 weeks or more and you have 3 or more of the risk factors listed below
- You gave birth less than 6 weeks ago and you have 2 or more risk factors listed below
- Body mass index of more than 30 kg/m2 before pregnancy or in early pregnancy (A BMI calculator is available on the Terveyskirjasto website)
- Age over 35 years
- Gene mutation that affects blood clotting, such as heterozygous FV (Leiden) gene mutation, heterozygous FII / prothrombin gene mutation G20210A
- A known venous or pulmonary embolism in a first-degree relative
- More than 3 previous pregnancies/births
- Smoking
- Pre-eclampsia
- A multiple pregnancy
- Immobility (paraplegia)
- Large varicose veins in the lower limbs
- Fetal death in the current pregnancy
- Caesarean section in the current pregnancy
- Preterm delivery (less than 37 weeks pregnant)
- Pregnancy from artificial insemination
3. Do you have a fever (over 38 degrees Celsius) or shortness of breath or a cough that worsens your general health and you have been mostly in bed rest for more than 3 days?
If you have a fever or shortness of breath or a cough that worsens your general health and
- you have been in bed rest for more than three days, and
- you are at a significant risk for thrombosis (list below), and/or
- you have three or more other risk factors for thrombosis (list below),
go to the COVID-19 page matching your home municipality's wellbeing services county for instructions and contact information for potentially starting heparin medication. The assessment for heparin medication needs to be completed within a few days. Please note that the Medical Helpline 116117 does not handle matters related thrombosis risk assessment.
Have you been diagnosed with one (1) risk factor from this list?
- Previously had a venous or arterial blood clot
- Thrombosis susceptibility (such as: APC resistance, FV Leiden gene mutation, prothrombin gene mutation G20210A, antithrombin and protein C or S deficiency, essential thrombocytosis, polycythemia vera, myeloma, PNH disease, phospholipid antibody syndrome)
- Active cancer or malignant blood disorder
- Cancer treatments (chemotherapy, radiation and hormonal therapy)
- Multiple injury or recent spinal cord injury
- Major surgical procedure less than one (1) month ago and/or current bed rest as a result.
Have you been diagnosed with three (3) or more of the thrombosis risk factors listed below?
- Cardiovascular disease, cardiac insufficiency
- Autoimmune disease (incl. Crohn’s disease, colitis ulcerosa, vasculitis, rheumatoid arthritis)
- Immobilisation / paralysis / using a wheelchair
- Closed lower limb cast
- Clozapine medication
- Recent surgery (<6 weeks ago)
- Diabetes
- Fatty liver
- 60 years of age
- Body mass index of more than 30 kg/m2 (A BMI calculator is available on the Terveyskirjasto website)
- Smoking
- Contraceptive pills, oral hormone replacement therapy
- Severe chronic kidney disease
- Lower limb venous insufficiency
- Central venous catheter
